The Sunroom Psychiatry Rates
We are currently in network and accepting Blue Cross and Blue Shield PPO for our psychiatric mental health services.
We also work with out-of-network insurance providers and upon request we can provide you with a superbill that can be submitted via your patient portal. Payment reimbursement from your insurance company will go directly to you. We recommend asking your insurance provider if they cover out of network costs. Patients are responsible for paying our fees up front if participating out-of-network or private pay.
This form is provided as a way to make the process of getting accepted into the practice more simple and efficient, but it can only be used if you agree that providing this information does not mean any doctor/therapist-patient/client relationship is being formed, and that no guarantee is provided that you will indeed be accepted into the practice.
